Independence recently introduced a program where providers can earn an incentive by closing gaps in care by rendering applicable services to eligible Medicare Advantage HMO and PPO patients for whom we have no claim or encounter data recorded in 2017.
Eligible providers were sent a letter from Independence describing this incentive along with a list of their Independence patients who need services for one or more of the following Star Rating Program* measures:
- osteoporosis management in women
- breast cancer screening
- colorectal cancer screening
- diabetic eye exam
- diabetic kidney disease monitoring
- rheumatoid arthritis management
- statin use in persons with diabetes
- statin therapy for patients with cardiovascular disease
An incentive is earned for each identified member once the required service has been completed and a claim or encounter has been submitted for the office visit satisfying the measure.
Encourage Independence patients to receive missing service(s)
Independence has created a template letter that physicians can mail to their Independence patients to notify them of the exam(s) and/or preventive service(s) they are missing. The letter is intended to encourage your Independence patients to make an appointment with you to receive the missing service(s). Providers can download the template letter online, customize it for their office/patient, and print it on their office letterhead.

*The Star Rating Program was developed by the Centers for Medicare & Medicaid Services to measure health plan quality. Ratings are published annually to help educate consumers prior to enrollment decisions.
