The Clinical Care Report, now available through the NaviNet? web portal, is a
useful tool that provides a summary of care your patients have received, based
on IBC-paid medical and prescription drug claims for each individual member. It
is not a complete record of your patient's care.
The Clinical Care Report can assist the care team in delivering personalized
and coordinated care to their patients. For example, it can be used in
pre-visit planning* to identify care that occurred since the patient's last
visit and provide insight into gaps in care. As pre-visit planning is also a
core component in the Patient-Centered Medical HomeTM (PCMH) model,
it can aid practices in gathering relevant information.
Information available on the Clinical Care Report includes the following:
disease conditions reported in the last two years;
visits to the emergency room in the past year;
hospital admissions in the past four years;
outpatient procedures in the past two years;
specialists seen in the past two years;
prescriptions filled in the past six months;
alerts by condition (i.e., gaps in care) if any;
lab tests with results (when available);
diagnostic imaging in the past two years;
immunizations in the past four years.
