Independence health rewards is an incentive program offered to Medicare
Advantage HMO and PPO members. The goal of the program is to incent members to
receive important screenings and other preventive services from their primary
care provider.
How it works
Medicare Advantage HMO and PPO members were sent a letter in June with
information about the program and instructions on how to register. The letter
included information explaining how the program works, how to report
activities, and how to qualify for and redeem rewards. The following grid
outlines the services eligible for rewards:

When members complete applicable recommended health care activities, they
can document them online, by mail, or by phone. Rewards can be redeemed as gift
cards to a wide variety of retailers where they can be redeemed. Members will
receive their reward in the mail approximately four to six weeks after
submission for activities reported by mail or phone. Activities reported online
can be redeemed for an instant e-gift card.
