The address of the new building is: AmeriHealth New Jersey 259 Prospect Plains Road, Building M Cranbury, NJ 08512 Facility and ancillary providers submitting key practice information changes (e.g., mailing address or practice name) will need to send such requests to the new address. Professional providers should continue to submit changes using the Provider Change Form. Note:Only the physical address of the AmeriHealth New Jersey office location will change.
