• Connections^SM Health Management Program. Through Connections, condition management is available 24/7/365 to eligible Pennsylvania members for common chronic conditions such as asthma, diabetes, COPD, and hypertension.
• Case management. Through a Health Coach, case management provides support to Pennsylvania members who are experiencing complex health issues or challenges in meeting their health care goals.
