Sometimes patients may need extra support when they
are dealing with an acute illness, chronic condition, or
multiple comorbidities, such as diabetes, congestive
heart failure, and asthma. In times like these, they often
look to you, their physician, for support.
At AmeriHealth New Jersey, our registered nurse case
managers are available to help you in your care for
these patients. They can work in collaboration with you
to support the plan of care you have created and the
medical treatment goals you have established.
By working together with you and your patients, our case
managers can:
- educate patients about their disease, condition, or
medications and offer resources to help them better
understand how to manage their health;
- assess patient barriers and determine alternate ways to
follow through with the plan of care you have created;
- advise on the importance of follow-up care and assist
in making appointments;
- teach patients about medication adherence and stress
the importance of bringing all medications to every
medical appointment;
- help patients navigate the health care system;
- refer patients to other Plan programs or benefits-related
resources, such as nutrition counseling or mental health
services;
- provide information on community resources.
