Facility and ancillary providers submitting key practice information changes (e.g., mailing address or practice name) need to send such requests to the new address. Professional providers should continue to submit changes using the Provider Change Form, which is available on NaviNet or on our website at www.amerihealth.com/providerforms.

Note: Only the physical address of the AmeriHealth New Jersey office location has changed. Most operational addresses (e.g., claims submissions) remain the same.
