The payer ID grids contain valuable information to assist you in claims submission, including alpha prefixes, payer information, and claims mailing addresses by product.

We are in the process of updating the professional and facility payer ID grids to reflect new products for 2016. Please be sure to download the most current versions, which will be available on our websites later this month as follows:

• AmeriHealth New Jersey. Go to the Provider Forms web page. Under Claims Requirements and Communications, select either Facility Payer ID Provider Number Reference or Professional Payer ID Provider Number Reference.
• AmeriHealth Pennsylvania. Go to the Electronic Data Interchange (EDI) web page. Under EDI Resources, select either Facility Payer ID or Professional Payer ID.
