The professional and facility payer ID grids contain valuable information to
assist you in claims submission, including prefixes, payer information, and
claims mailing addresses by product. The grids have recently been updated to
reflect product changes for 2018.
Please be sure to download the most current versions, which are available on
our websites as follows:
- AmeriHealth New Jersey. Go to the Provider Forms web page. Under Claims Requirements and
Communications, select either Facility Payer ID Provider Number Reference
or Professional Payer ID Provider Number Reference.
- AmeriHealth Pennsylvania. Go to the Electronic Data Interchange
(EDI) webpage. Under EDI Resources, select either Facility Payer ID
or Professional Payer ID.
