The 2012 release of the SMART^? Registry, which was mailed to provider?s offices in July, announced the launch of the Connections Provider Portal, a website for network clinicians. The Connections Provider Portal is accessible 24 hours a day, 7 days a week, and is designed to make it easy for you and your office staff to view patient reports, refer an eligible patient to a Health Coach, and link to patient education materials and physician support information. By logging on to the Connections Provider Portal you can: view and download your SMART Registry reports; access health coaching and Shared Decision-Making^? information; refer patients to a Health Coach at no cost to them; link to up-to-date clinical guidelines, patient education materials, and physician support information; print and download individual, detailed patient profile reports; make inquiries and provide feedback about the Connections program.
