The Centers for Medicare & Medicaid Services (CMS) announced a change in the contact information for Beneficiary & Family Centered Care (BFCC) Quality Improvement Organization (QIO) Program contractors effective June 8, 2019. Therefore, providers will need to include new contact information when completing the Notice of Medicare Non-Coverage form (Form CMS 10123) or the Important Message from Medicare form (Form CMS-R-193).
