The professional and facility payer ID grids contain valuable information to assist you in claims submission, including prefixes, payer information, and claims mailing addresses by product.

The grids have recently been updated to reflect product changes for 2019. Please be sure to download the most current versions, which are available on our Provider Forms web page. Under Claims Requirements and Communications, select either Facility Payer ID Provider Number Reference or Professional Payer ID Provider Number Reference.
