At AmeriHealth HMO, Inc. and AmeriHealth Insurance Company of New Jersey (collectively, AmeriHealth), our Registered Nurse Health Coaches (Health Coaches), licensed Behavioral Health Case Managers, and licensed Social Workers are here to help your patients (our members) enrolled in an AmeriHealth health plan* by coordinating care, providing information, and sharing community resources.
Coordination of care
Our integrated behavioral health and physical health care management programs cohesively support the whole person, including behavioral health, substance use, preventive health, a new diagnosis, and/or managing chronic or complex health situations. We address each person's medical condition along the health care continuum, considering all perspectives and providing longitudinal support.
How we support you and your patients
Our highly skilled Health Coaches, licensed Behavioral Health Case Managers, and licensed Social Workers assist your practice through:
- Resource coordination and assistance: We connect your patients with community resources not covered by insurance, such as medication assistance, transportation services, food resources, and home modification programs. We also assist with scheduling appointments and transitioning to alternative care when benefits cease.
- Comprehensive health and education: We address behavioral health and substance use needs and educate your patients on medication and plan-of-care adherence, managing new or chronic conditions, and shared decision-making.
- Patient support and communication: We assist with closing care gaps, supporting your patients post-discharge, encouraging them to discuss concerns and questions with their health care providers, and reporting any medication discrepancies and unreported needs, such as potential home care.

- To refer your patient (an AmeriHealth member) to a Health Coach, please complete the online Case and Condition Management Physician Referral Form or call 1-800-313-8628 and select prompt 2.
- To refer an AmeriHealth member for Behavioral Health Case Management, please call 1-844-505-2500.
*AmeriHealth members who are covered through fully insured employer groups are automatically considered eligible for health coaching. Members covered through certain self-insured employer groups may not be eligible for all components of health coaching, including condition management. Members can call Customer Service at 1-888-YOUR-AH1 (1-888-968-7241) (in New Jersey) or 1-800-275-2583 (in Pennsylvania) to verify their eligibility.
