Issue with processing claims with authorizations that include dosing and frequency detail

We have identified an issue when processing claims with an authorization for certain services that include dosing and frequency detail, such as infusion services and home health. Some claims are incorrectly rejecting for no authorization on file.

There is no action needed at this time. We are working to identify the root cause and implement a solution. We are updating the affected authorizations and will reprocess the claims.
