Dosage and Frequency Program expands to add five drugs

​Effective August 24, 2026, the AmeriHealth HMO, Inc. and AmeriHealth Insurance Company of New Jersey (collectively, AmeriHealth) and AmeriHealth Administrators Dosage and Frequency Program will expand to include five new drugs.

Drug additions

The following drugs will be added to this program: 

  • Darzalex® / Darzalex Faspro®
  • Imfinzi®
  • Keytruda® / Keytruda QLEX™
  • Opdivo® / Opdivo Qvantig®
  • Tecentriq® / Tecentriq Hybreza™

Review dosage and frequency requirements

During precertification, AmeriHealth and AmeriHealth Administrators verify that drug regimens meet the U.S. Food and Drug Administration (FDA) or company-recognized compendia. For specific requirements on each drug's dosage and frequency guidelines, refer to the drug's medical policy in the Medical and Claim Payment Policy Portal.
