Effective January 1, 2022, FutureScripts, our pharmacy benefits manager, will make its standard biannual changes to its Premium Formulary.
The Premium Formulary is a restricted formulary managed by FutureScripts available to our commercial members in Pennsylvania. This formulary includes all therapeutic categories and provides physicians with prescribing options. In addition, the formulary is reviewed biannually to add new drugs and to remove brand-name drugs when a generic equivalent becomes available.
How members benefit
When you prescribe a formulary medication, commercial members receive the drug for a lower copayment. If you prescribe a drug not on the formulary, the member's out-of-pocket cost will be greater. Prescribing generic drugs allows our commercial members to always pay the lowest copayment.
You may request a formulary exception if you wish to prescribe a drug for one of our members that is not on his or her formulary. If the formulary exception is granted, the member will pay the cost-sharing amount attributable to drugs that are on the formulary.
