08.01.32f: Pegfilgrastim (Neulasta^®) and related biosimilars. Notification date, 11/9/2021; Effective date, 2/7/2022.
08.01.73a: Filgrastim (Neupogen^®) and related biosimilars, and tbo-filgrastim (Granix^®). Effective date, 11/22/2021.
